ATTENTION STUDENT ATHLETES

 SCHOOL YEAR 2012-2013

All students must have a physical exam BEFORE trying out for any

sport at Eli Terry Jr. Middle School and Terryville High School. If your child

is planning to participate in a sport, please schedule the appointment for

the physical before school / practice starts. Physicals may be done by

your own doctor or by the school doctor. The following are the only dates

the school doctor will be doing sport physicals for the entire school year,

by appointment only.

Wednesday, August 15th from 8:00am - 1:00pm

Friday, August 17th from 8:00am - 1:00pm

Tuesday, November 6th from 8:00am - 11:00am

Tuesday, March 12th from 2:30pm – 4:30pm.

Students should report to Terryville High School for their physicals.

Students must have a sports form completed and signed by a parent in order

to be examined by the school doctor. To schedule an appointment,

at 860-314- 8004.

ATTENTION ALL SIXTH AND TENTH GRADE STUDENTS:

Blue physical forms, required by the State of Connecticut for all 6th

and 10th grade students and completed by your own doctor, will be

acceptable for sport participation for the entire year.
